The waiting list opens at 9:00 a.m. on April 1, 2020 and will remain open until 150 eligible applications have been taken.  Applications will be taken on-line only at

Only one person per household may apply. 

Eligible applicants will be placed on the waiting list according to date time stamp of application.


  1. You must be a United States Citizen, or a noncitizen with eligible immigrant status.
  2. You must provide a social security number and birthdate for each family member.
  3. You must meet eligibility requirements for Mainstream vouchers:
  4. A member of the household must be between the ages of 18-62 and must have a developmental, mental, emotional or physical disability


  • You must meet ONE of the Mainstream Targets:
  • Transitioning out of institutional or other segregated settings; or
  • At serious risk of institutionalization due to lack of community supports; or
  • Experiencing homelessness; or
  • Will imminently lose prime-time residence; or
  • Are fleeing or attempting to flee domestic violence; or
  • Living in permanent supportive housing or rapid rehousing projects and no longer need that level of service.
  • The household’s gross income must be equal to or less than:
